THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Who We Are

Valitide LLC operates a telehealth platform that connects patients with licensed healthcare providers for the evaluation and prescription of GLP-1 receptor agonist medications and related treatments. This notice applies to the health information collected and maintained through the Valitide platform at valitide.com.

How We May Use and Disclose Your Health Information

For Treatment

We may use and disclose your health information to coordinate your care. For example, we share your medical intake information with the licensed provider assigned to evaluate your case, and we may transmit your prescription to a pharmacy if treatment is prescribed.

For Payment

We may use and disclose your health information as needed to process payments for services. We minimize PHI in payment transactions — our payment processor receives only a transaction amount and a non-identifying reference number.

For Healthcare Operations

We may use and disclose your health information for internal operations such as quality improvement, auditing, and compliance activities.

Other Permitted Disclosures

We may also disclose your information without your authorization for the following purposes as permitted or required by law:

  • As required by federal, state, or local law.
  • For public health activities (e.g., reporting adverse drug reactions).
  • To avert a serious threat to health or safety.
  • For health oversight activities (e.g., audits, investigations).
  • In response to a court order, subpoena, or law enforcement request.
  • To coroners, funeral directors, and organ donation organizations as required.
  • For workers' compensation claims as required by law.

Uses Requiring Your Authorization

We will not use or disclose your health information for the following purposes without your written authorization:

  • Marketing purposes
  • Sale of your health information
  • Most uses of psychotherapy notes (if applicable)
  • Any other purpose not described in this notice

You may revoke any authorization in writing at any time, except to the extent we have already taken action in reliance on your authorization.

Your Rights Regarding Your Health Information

Right to Access

You have the right to inspect and obtain a copy of your health information maintained by Valitide. We will respond to your request within 30 days. You may submit requests through your patient dashboard or by emailing privacy@valitide.com.

Right to Amend

You have the right to request that we amend your health information if you believe it is inaccurate or incomplete. We may deny your request in certain circumstances but will provide a written explanation.

Right to an Accounting of Disclosures

You have the right to request a list of certain disclosures of your health information that we have made. This does not include disclosures for treatment, payment, or healthcare operations.

Right to Request Restrictions

You may request restrictions on how we use or disclose your health information. We are not required to agree to your request unless the disclosure is to a health plan for payment purposes and the health information relates solely to a service you paid for in full out of pocket.

Right to Confidential Communications

You may request that we communicate with you about health matters through a specific method or at a specific location. For example, you may request that we only contact you by email rather than phone.

Right to a Paper Copy

You have the right to obtain a paper copy of this notice upon request, even if you agreed to receive it electronically.

Our Responsibilities

  • We are required by law to maintain the privacy and security of your health information.
  • We will notify you promptly if a breach occurs that may have compromised your health information.
  • We must follow the terms of this notice currently in effect.
  • We will not use or disclose your health information without your authorization except as described in this notice.

Changes to This Notice

We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as any information we receive in the future. The current notice will be posted on our website at valitide.com/npp.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
